It is easily accessible from the Taipei Metro's Chiang Kai-Shek Memorial Hall Station. Map
The machine is located under the verandah of the National Theater, in the row of vending machines by the retail area entrance facing the National Concert Hall. This retail area can be entered through the openings on either side of the set of stairs facing the National Concert Hall. No admission is required.
The hand-crank machine requires 30 NTD (about 1 USD) in the form of three 10 NTD coins to press a provided copper token. While the first two designs roll beautifully, the third design seems to roll a bit short.

Need scan/ photo of designs on a plain white background.
Retired 1 designs were:
1) the Chiang Kai-Shek Memorial hall along with the abbreviation “CKSMH” and "?????", its Chinese name
2) the Gate of Great Centrality and Perfect Uprightness along with “CKSMH” and”?????”; the inscription on the gate reads (from right to left) "????", but on the actual gate, it has since been replaced with "????", meaning Liberty Square
3) the National Chiang Kai-shek Cultural Center along with the abbreviation NTCH referring to the National Theater and Concert Hall and "?????”, its designation in Chinese
1/25/14: The old 3 design machine is gone. There is a new machine with 4 designs at the same location. The designs are very similar with the old machine.
